Lost or Stolen Device

Procedure: University of Minnesota Duluth

If your University-owned computer or other device was lost or stolen, follow these steps to help secure your and the University's data on the device.

Step 1: File a Police Report

You will need to get an official case number from the police:

  • If the incident occurred on the Duluth campus:
    • Contact the University of Minnesota Duluth Police Department (UMD PD) at 218-726-7000.
  • If the incident occurred outside the Duluth campus:
    • Seek out the local police at your location to report the loss.

Step 2: Notify University Information Security (UIS)

Take the following steps within 24 hours of the incident:

  1. Download and fill out the UMN Private Data Form for Security Incident Response.
  2. Email the form with the following information to [email protected]:
    • A brief description of the incident.
    • The police report case number acquired from Step 1.

Step 3: File an Insurance Claim

The University has an Electronic Data Processing (EDP) insurance policy that provides replacement coverage for University-owned damaged or stolen desktops, laptops, projectors and tablets (e.g., iPads). The departmental deductible for this policy is $500 per occurrence.

Your department will be able to apply insurance payout funds to replace the device after you take these steps:

  1. Download and fill out the Property Loss Notice – Laptop Computer Program Form (UM1532).
  2. Email the completed form to the Office of Risk Management at [email protected].

Step 4: Opening a Ticket With ITSS Help Desk

Finally, if you have not already, contact the ITSS Help Desk to open a ticket so that the appropriate IT support unit can work with you to secure your data and replace your device.

Please include the following in your ticket:

  1. The police report case number acquired from Step 1.
  2. The security ID string in the auto-reply from [email protected] from Step 2.
    • An ID string in the form of [UMN Security #XXXXXX] will be generated for you to track your incident. Include this ID string in the title of all correspondence related to your incident.
  3. Attach the completed Property Loss Notice Form from Step 3.
